docs(readme): fix platforms, doc links, org slug; drop stale sections

Browse source 
- add Telegram and Discord to Supported Platforms (3x4 grid)
- point documentation links at docs.trypost.it (was trypost.it/docs)
- correct the GitHub org slug trypost-it -> trypostit (broken links)
- drop the "TryPost 1.0 is here" announcement and the non-existent Roadmap link
